Diabetic retinopathy is damage to the tiny blood vessels in the retina, the light-sensitive tissue at the back of the eye responsible for capturing images and sending them to the brain. This damage occurs due to prolonged high blood sugar levels, leading to weakening and damage of these vessels, leakage of fluids or blood from them, or even their blockage.

Late Stages and Serious Complications

Initially, diabetic retinopathy may not show obvious symptoms, making regular check-ups essential for early detection. As the condition worsens, serious complications begin to appear, which include:

1. Vitreous Hemorrhage

One of the most dangerous complications of diabetic retinopathy is vitreous hemorrhage. This occurs when new, weak, and abnormal blood vessels (called neovascularization) grow on the surface of the retina or in other areas of the eye. These vessels are very fragile and can rupture easily, leading to blood leakage into the vitreous body, the jelly-like substance that fills the eyeball. This bleeding can range from small, floating spots in the field of vision to complete blockage of vision, causing sudden and severe vision loss.

2. Retinal Detachment

In advanced cases, the growth of fibrous tissue and fibrosis associated with new blood vessel formation can pull the retina away from the back wall of the eye. This is known as tractional retinal detachment. This detachment can lead to permanent vision loss if not surgically treated immediately. This type of detachment is a severe complication that requires urgent surgical intervention.

3. Glaucoma (High Eye Pressure) or Neovascular Glaucoma

When new blood vessels form in the iris (the colored part of the eye) or in the angle of the eye that drains fluid, it can block the eye's natural fluid drainage system. This blockage causes a sharp increase in eye pressure, a condition known as neovascular glaucoma. This type of glaucoma is extremely destructive and can quickly damage the optic nerve, leading to irreversible vision loss.

4. Diabetic Macular Edema (DME)

Diabetic macular edema is one of the main causes of vision loss in people with diabetes. It occurs when fluids and fats leak from damaged blood vessels in the retina into the macula, the central area of the retina responsible for detailed and central vision (such as reading and recognizing faces). The accumulation of fluid causes swelling of the macula, leading to distorted vision, blurriness, and difficulty with central vision.

5. Non-Proliferative vs. Proliferative Diabetic Retinopathy

Diabetic retinopathy can be divided into two main stages: non-proliferative (NPDR) and proliferative (PDR). In the non-proliferative stage, existing blood vessels are damaged and leak fluids. In the proliferative stage, which is the more serious stage, the eye begins to form new, weak, and abnormal blood vessels. This stage carries the greatest risk for serious complications such as vitreous hemorrhage and retinal detachment.
