Lifespan of Intraocular Silicone Oil and Risks of Permanent Retention
What happens if intraocular silicone oil emulsifies or degrades? Dr. Mahmoud Hassan explains the risks of leaving it in for more than 6 months and how to safely remove it.
The Lifespan of Silicone Oil Inside the Eye: When Should It Be Removed?
Injecting silicone oil into the eye is a crucial surgical procedure resorted to in certain complex retinal diseases, such as severe retinal detachment, after complex retinal surgeries, or in cases of severe intraocular hemorrhage that prevents clear vision. Silicone oil acts as a "tamponade" or "scaffold" that helps to hold the retina in place and prevent it from detaching again, allowing it a chance to heal and recover. However, a question many patients ask is: What is the lifespan of silicone oil inside the eye? And can it be left permanently?
In reality, silicone oil is not considered a permanent substance inside the eye; rather, it is a temporary material used as a therapeutic tool. The lifespan of silicone oil inside the eye varies based on several factors, most importantly the type of oil used, the condition of the eye, and the stability of the retina. Generally, it is recommended to remove silicone oil after a period ranging from 3 to 12 months after injection, once the retina's stability and sufficient healing are confirmed. This timing is determined by the retinal surgeon with great care after conducting the necessary examinations.
Why is it not recommended to leave silicone oil inside the eye for long periods?
Leaving silicone oil inside the eye for periods longer than recommended carries numerous potential risks and complications that could negatively affect the eye's health and vision in the long term. Scientifically, silicone oil, despite being a relatively inert substance, is not entirely free from potential interactions with the internal ocular environment.
- Oil Emulsification: Over time, silicone oil may undergo a process called "emulsification." This process means that the oil begins to break down into very small droplets, which can then migrate to other parts of the eye, such as the anterior chamber or the lens. This migration can cause chronic inflammation, elevated intraocular pressure, and corneal damage, significantly impacting vision.
- Elevated Intraocular Pressure (Glaucoma): The emulsification and migration of oil can block the drainage pathways of aqueous humor from the eye, leading to acute or chronic elevation of intraocular pressure. If this elevated intraocular pressure is not managed, it can result in permanent damage to the optic nerve, known as glaucoma, and may progress to permanent vision loss.
- Cataract Formation: The formation of cataracts (opacification of the eye's natural lens) is a common complication of leaving silicone oil in place for prolonged periods. The oil can come into contact with the eye's natural lens and cause a change in its structure, leading to its clouding and loss of transparency, and consequently, decreased vision. Often, patients may require cataract surgery after previous retinal detachment surgeries and silicone oil injection.
- Chronic Eye Inflammation: The prolonged presence of silicone oil, especially if it has emulsified, can lead to chronic inflammation inside the eye, causing persistent pain, redness, blurred vision, and potentially affecting the efficacy of other treatments.
- Effect on the Retina Itself: Although silicone oil is used to support the retina, its presence for very long periods can lead to chronic changes in the retina itself, such as the formation of fibrous tissues (PVR) that may be difficult to remove later, or may affect its long-term function.
When should silicone oil removal be considered?
Patients should adhere to regular follow-up appointments with their retinal surgeon after silicone oil injection. Signs that warrant immediate evaluation include:
- A sudden increase in eye pain or redness.
- Appearance or increase in floaters or light flashes.
- Persistent blurred vision or a noticeable decrease in vision.
- A sensation of increased pressure in the eye.
The timing of silicone oil removal is usually determined based on retinal stability, but if any of these symptoms appear, a doctor should be consulted immediately. The removal of silicone oil from the eye is a necessary surgical procedure to restore normal vision and prevent long-term complications.
My Experience with Silicone Oil: Dr. Mahmoud Hassan's Advice
As a specialized vitreoretinal surgeon, I emphasize that silicone oil is a valuable therapeutic tool, but not a permanent solution. Its primary purpose is to give the retina a chance to heal. Therefore, adhering to follow-up appointments with the doctor, meticulously following instructions, and not hesitating to inform the doctor of any changes in the eye are all crucial for ensuring the best possible outcomes.
At Al Mashreq Eye Center in Cairo, we adhere to the latest international protocols in managing retinal cases that require silicone oil, from injection to removal. We fully understand the importance of these steps in preserving eye health and vision.
It is worth noting that the approximate cost of silicone oil removal from the eye in Egypt for 2026 is around 25,000 EGP (approximate). These prices are estimates and depend on the patient's individual condition and the complexity of the procedure.

Frequently Asked Questions
