Proliferative Diabetic Retinopathy: A Dangerous Journey from Sugar to Vision Loss
Diabetes is a chronic disease that requires precise and continuous monitoring. In addition to its known effects on the heart, kidneys, and nerves, diabetes poses a silent threat to one of our most important senses: vision. When blood sugar levels remain elevated beyond normal limits for prolonged periods, the tiny blood vessels in the retina begin to suffer gradual damage, leading to a condition known as diabetic retinopathy. Perhaps the most prominent feature of this retinopathy is its progression through stages, which may start subtly and unnoticed, but can worsen to reach the stage of proliferative diabetic retinopathy, which is the most dangerous stage and can lead to permanent vision loss if appropriate therapeutic intervention is not provided in a timely manner.
Early Stages: The Silent Warning
Initially, a diabetic patient may not experience any obvious symptoms, as the retina begins to show minor changes. These changes include:
Microaneurysms: Small bulges in the walls of blood vessels.
Dot Hemorrhages: Small bleeding within the retina.
Hard Exudates: Leakage of fats from damaged blood vessels.
At this stage, central vision may not be affected, but a regular fundus examination by a specialist doctor is the only way to detect these changes early. Periodic fundus examination for diabetic patients is vital for early detection of these changes.
Intermediate Stage: Beginning of Vision Impairment
As blood sugar levels continue to rise, the damage worsens. Blood vessels begin to deteriorate further, and some may become blocked, depriving parts of the retina of the necessary oxygen and nutrients. At this stage, signs appear such as:
Intraretinal Hemorrhages: Larger and more widespread bleeding.
Neovascularization: A hallmark indicating that the retina is in a state of severe oxygen deprivation. These new vessels are fragile and brittle, making them prone to bleeding easily.
Onset of Diabetic Macular Edema (DME): This is the accumulation of fluid in the central area of the retina responsible for detailed vision, leading to blurred and distorted vision.
At this stage, the patient may begin to notice some changes in the quality of their vision, such as difficulty reading or seeing fine details. Here, therapeutic intervention becomes more urgent.
Advanced Stage: Proliferative Diabetic Retinopathy (PDR)
This is the most dangerous stage, where new, abnormal blood vessels proliferate significantly in abnormal locations, such as the surface of the retina or even inside the vitreous body (the jelly-like fluid that fills the eyeball). These new vessels are the main cause of severe complications:
Severe Vitreous Hemorrhage: When new blood vessels rupture, profuse bleeding occurs inside the vitreous body. This bleeding can completely obscure vision, causing the patient to experience dense floaters or a fog obstructing vision.
Fibrovascular Proliferation: Fibrous tissues begin to grow around the new blood vessels, which can lead to traction on the retina, pulling it away from its normal position.
Retinal Detachment: If the traction from the fibrous tissues continues, it can lead to retinal detachment, an emergency condition that requires immediate surgical intervention to prevent permanent vision loss.
Neovascular Glaucoma: New blood vessels can lead to blockages in the eye's fluid drainage system, causing a sharp rise in intraocular pressure and damage to the optic nerve, a condition known as glaucoma.
At this stage, permanent vision loss becomes a real threat if treatment is not administered quickly and effectively. Neglecting this stage often means facing significant difficulties in restoring vision.
When Does Diabetes Cause Permanent Vision Loss?
Diabetes causes permanent vision loss when the complications of proliferative diabetic retinopathy reach advanced, untreatable stages, or when warning signs and symptoms are ignored, and specialized medical help is not sought in a timely manner. The main causes of permanent vision loss in this case include:
Recurrent vitreous hemorrhage that does not stop and leads to severe fibrosis.
Retinal detachment that was not surgically treated in time, leading to permanent damage to retinal cells.
Development of neovascular glaucoma causing irreversible damage to the optic nerve.
Severe and cicatricial fibrosis that pulls the retina and makes it irreparable.
Treatment and Prevention: Hope in Facing the Challenge
Fortunately, effective treatment options can help slow the progression of the disease, and in some cases, improve vision and prevent permanent vision loss. Treatment depends on the stage of the disease, and available options include:
Laser (Photocoagulation): Laser is used to cauterize abnormal new blood vessels and reduce the risk of bleeding. Argon laser is an effective treatment in advanced stages.
